LOCATION OF SERVICE DELIVERY: South Huron County, including Grand Bend, Bayfield, Zurich, Exeter and surrounding rural areas
HOURS OF SERVICE: Approximately 1 to 4 hours per week (more or less, based on clients needs)
CLIENT DEMOGRAPHICS: Support to individuals who are facing a life-threatening illness or those who are bereaved, in their homes, hospitals and retirement home, long term care facility or hospital.
SERVICE COMPONENTS:
* Screened and trained volunteers are matched with clients on a one-to-one basis and offer volunteer visits in the home, hospital or long term care homes
* Volunteers offer companionship, encourage client interests/activities, planned outings and relief for caregivers
* Bereavement Consultant offers bereavement follow-up visits, support groups, resources, books, videos, and pamphlets
* RAINBOWS Support Groups: RAINBOWS is a program that provides support groups for children and adolescents who have experienced a painful transition within their family such as death, divorce, becoming a member of a single-parent home, foster home or step/blended family. The primary goal of RAINBOWS is to provide a safe, supportive environment for these young people to sort through their pain and confusion, build esteem, and learn coping skills to help them deal with their losses.
USER FEES: There is no cost to clients
OFFICE HOURS: Monday, Tuesday and Thursday: 9 a.m. to 5 p.m. An answering machine is in operation during off hours.
