+/- Blood Pressure Clinics in North Perth

VON’s Wellness for Seniors initiative introduces Cardiovascular Health Awareness Program + Action Plan (CHAP+AP).  It is a community-based program that brings together local family physicians, pharmacies, allied health professionals, public health representatives, volunteers, and community organizations to work as partners to promote and actively participate in the prevention and management of cardiovascular disease and stroke.

The goal of CHAP+AP is to implement a peer health educator program of cardiovascular health awareness, including blood pressure monitoring in communities across Ontario and elsewhere.

CHAP+AP is made available free of charge to older adults in the community.  It offers opportunities for enhanced multiple blood pressure monitoring and other health promotion, prevention and management activities addressing modifiable risk factors for cardiovascular disease and stroke.

SERVICES PROVIDED: Footcare Clinics at location of customers choice, provided by Registered Nurses and Registered Practical Nurses, with basic & advanced footcare training.  Footcare for private-pay individuals and Veterans Affairs clients can be arranged by calling the VON Office.

SERVICE COMPONENTS:
* A minimum of 4 clients per Footcare Clinic
* Assessment and physical examination of the foot
* Foot care includes:
   (i)  Care and cutting of normal nails
   (ii) Callous care
   (iii) In-grown nail care
   (iv) Basic health information related to the anatomy of the foot, including differences related to age
   (v) Discussion related to high risk problems
   (vi) Client education related to buying shoes and characteristics of a good shoe
   (vii) Referral to physician / chiropodist

+/- Home Maintenace Referral Service

LOCATION OF SERVICE DELIVERY: Northern part of Perth County including Listowel, Atwood, Gowanstown, Molesworth and surrounding rural areas

CLIENT DEMOGRAPHICS:  Serving seniors or disabled adults living in the community who require assistance with light or heavy home maintenance jobs that are beyond their capabilities.

SERVICE COMPONENTS:  The Service will provide a linkage between workers who have been through a screening process for security purposes, and consumers who require home maintenance, seasonal cleaning and/or minor home repairs.

Consumers call the North Perth Community Support Services Office; they are briefed about the service and then are given the names and telephone numbers of three (3) appropriate workers.  Consumers then contact and arrange for service.

USER FEE:  There is no cost for the client to receive referral names and telephone numbers.

Clients are responsible to pay a mutually agreed upon fee directly to the worker.

Location:  Eligible individuals residing in the Municipality of North Perth

Hours of Service:  Hot meals are delivered to clients by volunteers, Monday to Friday (except Statutory Holidays) between 11:45 a.m. and 12:30 p.m.
Frozen meal delivery is available on Monday mornings, or arrangements can be made for pick-up during regular business hours at the VON North Perth Community Support Services office..

Client Demographics:  Seniors, adults with a disability, persons convalescing from illness or surgery, and person unable to care for their own nutritional needs.

Service Components:
* The purpose of the Meals on Wheels program is to provide hot or frozen nutritious meals
* Meals are delivered to client's homes by volunteers
* The program provides and promotes good nutrition
* Volunteer involvement provides regular social contact and a health and safety check
* Special diet requirements can be accommodated
* Referrals are made to other organizations when appropriate

User Fee:
 
Hot meals are $7.50 each - fees billed monthly
Frozen meals prepared by "Apetito" are $5.50 per entrée and $1.50 for soup or dessert - fees are payable at time of delivery/pick-up

LOCATION OF SERVICE DELIVERY:  South Huron County, including Grand Bend, Bayfield, Zurich, Exeter and surrounding rural areas

HOURS OF SERVICE:  Approximately 1 to 4 hours per week (more or less, based on clients needs)

CLIENT DEMOGRAPHICS:  Support to individuals who are facing a life-threatening illness or those who are bereaved, in their homes, hospitals and retirement home, long term care facility or hospital.

SERVICE COMPONENTS:
* Screened and trained volunteers are matched with clients on a one-to-one basis and offer volunteer visits in the home, hospital or long term care homes
* Volunteers offer companionship, encourage client interests/activities, planned outings and relief for caregivers
* Bereavement Consultant offers bereavement follow-up visits, support groups, resources, books, videos, and pamphlets

* RAINBOWS Support Groups:  RAINBOWS is a program that provides support groups for children and adolescents who have experienced a painful transition within their family such as death, divorce, becoming a member of a single-parent home, foster home or step/blended family.  The primary goal of RAINBOWS is to provide a safe, supportive environment for these young people to sort through their pain and confusion, build esteem, and learn coping skills to help them deal with their losses.

USER FEES:  There is no cost to clients

+/- SMART In-Home Exercise Program

(Seniors Maintaining Active Roles Together)

Hours of Service:  As agreed upon by the in-home exercise volunteer and the client

Client Demographics:
* individuals who are 55 years of age or older and a resident of North
   Perth; who are home-bound, or have limited access to the community 
   due to physical limitations, problems with transportation or cultural
   barriers; and would like to enhance their level of fitness in the comfort of
   their own home
* isolated seniors recovering from a health crisis who would benefit from
   and are willing to participate in a gentle exercise program

Service Components:
* trained in-home exercise volunteers make in-home visits a minimum of
   once a week to teach participants 10 simple, yet progressive exercises
* completion of participant information questionnaire
* participants keep a daily log of their exercise activity

User Fee:
The cost of the Program is $30.00 for 15 in-home visits, this works out to $2.00 per visit.

"Special Steps is a partnership program between the VON Perth-Huron and the Alzheimer Society of Perth County."

Location of Service Delivery: Perth County

Hours of Service:  Approximately 1- 2 hours per week (based on clients/caregivers wishes and/or needs)

Client Demographics:  Individuals diagnosed with Alzheimer Disease or another Alzheimer-type Dementia and are living independently in the community

Service Components:
* Volunteers receive 6 hours training, plus 4 hours in a VON Adult Day Service in which clients with Alzheimer Disease or another Alzheimer-type Dementia are in attendance.
* Volunteers are matched with clients on a one-to-one basis.
* Volunteers offer support and friendship to the clients through ongoing regularly-scheduled visits/walks.
* The walks can take place in the clients neighbourhood, schools or at local shopping malls.

Transportation:  Transportation of an occasional or social nature may be provided within the ongoing one-to-one relationship.

User Fee:  There is no cost to clients unless transportation is provided.

  • Accessible Mobility Bus & Van

Location: Eligible individuals residing in the Municipality of North Perth

Hours of Service:  Based on the needs of the clients and the availability of the volunteer drivers

Client Demographics:
* seniors, disabled adults and needy individuals living in the community who require transportation

Service Components:
* Accessible transportation (Mobility Bus & Van) provided by staff, volunteer drivers, using their own vehicles, provide transportation to seniors, disabled adults and needy individuals, who require transportation to appointments and other activities (in town or out of town)
* referrals are made to other organizations when appropriate

User Fee:   Fees are based upon mileage rate of $.37/km, plus any parking costs.  Accesible Transportation fees range from $4.00(one-way) to $.80/km for out-of-town trips.  Fees are billed monthly
