You will be responsible for assessing the nutritional needs, implementing and evaluating nutritional interventions in the home setting. You will serve as a liaison to the physician, home care coordinator, other health care professionals on the team, hospital dietitians and community resource representatives.

Qualifications:
- Current Registration with the College of Dietitians of Ontario
- Car and valid driver’s license
- Strong team-building skills
- Self-directed and able to take initiative
- Strong organization skills
- Excellent verbal and written communication skills
- Second language would be a definite asset
