Canadian Association for Community Care

The original lead partner, CACC, merged with CHA in December, 2006. CACC’s broad membership base included home care and home support agencies, meals programs, community groups, long term care facilities, Community Care Access Centres, regional planning bodies, provincial and national associations, corporations and individuals. The major activities of CACC included promoting information exchange among provinces/territories, producing training resources, carrying out practical research projects, and advocating on behalf of the community care sector.

 

Canadian Healthcare Association

CHA is the federation of provincial and territorial hospital and health organizations across Canada representing a broad continuum of services, including acute care, home and community care, long term care, public health, mental health, palliative care, addiction services, children, youth, and family services, and housing services. CHA is a leader in developing, and advocating for, health policy solutions that meet the needs of Canadians.

Canadian Pharmacists Association

Founded in 1907, the CPhA is the national professional voluntary association committed to providing leadership to pharmacists and improving the health of Canadians. Members are active in community and hospital pharmacies, in long term care facilities, home care, academia and industry. CPhA’s mission is to establish the pharmacist as the health professional whose practice, based on unique knowledge and skills, ensures optimal patient outcomes from drug therapies.
